Life Transitions Treats The Whole Person, Not Just An Illness
What Is Life Transitions?
Life Transitions is the name of the palliative care programs offered by Palliative CareCenter & Hospice of Catawba Valley. Life Transitions addresses the best way to manage and treat your physical symptoms caused by your illness. At the same time, Life Transitions addresses any social, emotional or spiritual needs you or your family may have. Life Transitions does not interfere with your relationship with your family doctor, but is designed to complement and aid the care you receive and any treatments your doctor has ordered.
Life Transitions considers you the leader of the decision making team in determining the care you need, and the support you and your family need.
Why Would I Want Life Transitions?
If you are dealing with an illness that limits your daily activities and you are not sure where to turn, a Life Transitions consultation may be what you need.
Who Would I Consult With?
You will meet with a specially trained physician or a nurse practitioner educated in palliative medicine. The Life Transitions team will discuss your illness and needs with you, and then pass along recommendations to your family doctor. You can discuss these suggestions with your doctor and then decide upon the best approach that will meet your specific wishes.
How Do I Arrange A Consultation?
Request that your doctor arrange a consultation. If you are in the hospital or in a facility, ask your nurse. If you prefer, call us directly at 828/466-0466, and we will take care of the arrangements.
If you and your personal physician decide that you would benefit from additional follow-up care, our Life Transitions staff, physicians, and nurse practitioners will design a specific program that best fits your needs.
